German-based healthtech company Oska Health announced an €11 million seed funding round, led by Capricorn Partners and SwissHealth Ventures. Other investors include BMH, LBBW Venture Capital, Revent, GoHub Ventures, and Aurum Impact. This funding confirms the growing confidence in a hybrid care model that has been sustainably improving the care of chronically ill people for over three years.
A large part of the care for chronically ill patients takes place not in the doctor's office, but in everyday life. It is precisely there, however, that the biggest gaps in care arise – especially for multimorbid patient groups suffering from several chronic diseases such as chronic kidney disease (CKD), diabetes, and high blood pressure. Around 50% of people with chronic conditions do not take their medication correctly, and about 80% fail to make lasting changes to their lifestyle. This not only impacts their quality of life but also costs them money: Around 70% of total healthcare expenditure in Germany is attributable to chronically ill patients, and this figure is rising.
In Germany alone, 9 million adults suffer from chronic kidney disease (CKD) , and across Europe it is 1 in 10 adults .
Oska Health provides continuous, personalized support from trained health consultants to help patients achieve lasting behavioral changes between doctor's appointments. Patients receive guideline-compliant, evidence-based support, grounded in behavioral science, through digital therapy – via video call, chat, or a dedicated app – to implement medical recommendations.
“Our healthcare system excels at diagnostics but is weak in providing everyday support,” says Niklas Best, CEO and co-founder of Oska Health. “We combine personalized care with AI to close precisely this gap. This helps patients actually implement their therapies – and thus relieves the burden on both doctors and health insurance companies. This finally makes high-quality care for chronically ill patients scalable.”
The model has become established in practice: More than 20 health insurance companies already work with Oska Health. The service is free for insured individuals. Health insurance companies benefit from improved care for high-risk patients requiring significant medical attention, as well as from the prospect of lower complication and hospitalization rates.
Doctors also report a noticeable reduction in their daily practice workload. Oska Health integrates seamlessly without additional software or effort and supports the implementation of medical recommendations in patients' everyday lives. "Many therapies fail not because of the medicine itself, but because of its implementation at home," says Dr. Sylvius von Borries, a general practitioner. "Oska Health closes precisely this gap, relieving the burden on medical staff and improving patient adherence."
With the new capital, the company plans to expand existing programs and broaden its network of health insurance companies and physician partners.
